ICC of Des Plaines Membership Form

In order to vote in any upcoming "General Body Meeting" you must:

1/download and sumbit the Membership form

2/Reside for at leat one year in the following areas:
Des Plaines
Elk Grove Village
Glenview
Morton Grove
Mount Prospect
Niles
Northbrook
Park Ridge
Prospect Heights
Rosemont
Wheeling


Note: The SS# on the form is not required.
